Onondaga County Child Protective Services The representative from Child Protective Services provides information about the child if there is an open case and educates the Team concerning the function and operation of the Onondaga County Department of Social Services and the State Central Registry (SCR).
NYS Office of Children and Family Services The designated representative of the NYS Office of Children and Family Services participates in any fatality review concerning the death of a child that was the subject of a State Central Registry report, a child for whom there is an open Child Protective Services or Preventive Services case, and/or a case relating to a child who died while in the care and custody or the guardianship of the Onondaga County Department Social Services or a voluntary authorized agency located in Onondaga County.
Onondaga County Health Department Division of Maternal & Child Health Madison County Health Department Preventive Services The designated representative of the Onondaga/Madison County Health Departments educates the Team concerning the role and function of the agency and provides definitions and insights concerning any medical condition relevant to the child fatality under review.
Onondaga County Mental Health The designated representative of Onondaga County Mental Health provides information on issues related to mental health diagnoses, suicide risk and appropriate mental health treatment.
Office of the Onondaga County Medical Examiner The designated representative of the Office of the Onondaga County Medical Examiner educates the Team concerning medical issues, terminology, concepts and practices, and instructs the Team regarding general procedures followed by the Office of the Medical Examiner in investigating a child's death. S/he also provides information on the status and results of a Medical Examiner investigation, including an explanation of cause and manner of death.
District Attorney The designated representative of the District Attorney educates the Team concerning applicable criminal law, the nature of criminal proceedings, and the prosecutor’s role and function. S/he also notes the status of a criminal case as to whether or not it can be discussed at a CFRT meeting.
Onondaga County Attorney The designated representative of the Onondaga County Attorney's Office provides information on legalities and the legal process followed in Family Court proceedings. S/he also provides any pending or prior court proceedings relevant to a particular child fatality.
Local Law Enforcement Agency and the State Police In conjunction with the District Attorney’s Office, this representative presents information and facts concerning law enforcement’s actual investigation of the child fatality to the extent such disclosure does not compromise the investigation, impede an arrest, or affect the integrity of the criminal prosecution or the presentment of the case in Family Court.
Emergency Medical Services The designated representative(s) from Emergency Medical Services provide information on details at the scene of the child fatality and medical information on the emergency procedures performed.
Pediatrician The Team pediatrician should have expertise in child abuse. S/he provides information on medical issues, medical terminology, and interpretation of medical records, documents and tests. S/he also provides expert opinions on medical evidence in a child fatality case.
Representatives from Crouse, St Joseph’s and University Hospitals The designated representatives of these hospitals provide information on the medical diagnosis and care of the child reviewed in a specific child fatality case.
Center for Maternal Child Health (C-Match) The representative from C-Match provides information on infant mortalities and reviews the prenatal, birth and infant health medical records of a specific child fatality case.
REACH CNY/NYS SID Center The Central New York Satellite Office of the New York State Center for Sudden Infant Death, housed within REACH CNY, Inc. in Syracuse, provides support and referrals to families whose infants die suddenly and unexpectedly in the Central New York region. It also provides educational programs to professionals and the community at large to increase public awareness and knowledge about SIDS, infant mortality and risk reduction methods. REACH CNY is a subcontractor of the statewide office of the New York State Center for SID at SUNY Stony Brook and is also a subcontractor of the Onondaga County Health Department.
McMahon/Ryan Child Advocacy Site The representative from the McMahon/Ryan Child Advocacy Site provides information on supporting and enhancing child advocacy in the community.
Case Specific Invitees The case specific invitee’s role is determined by the contact that person had with the deceased child, his/her siblings, other children in the home, parent(s) or other person(s) who were legally responsible for the child. It is expected the invitee will share information and documents with the Team to the full extent permitted under the law. Updated June 23, 2009
